Life Care Home Health Inc was established by a team of highly specialized health care professionals with years of experience in the home health setting and acute care setting with outstanding leadership in caring for the older adult. We strive to set the standard in home health care with an exceptional approach to personalized plan of care by understanding the uniqueness and special needs of each individual.

On behalf of our patients, our staff assists with reimbursements, verification of coverage from medicare and other insurance companies.  We work hand in hand with Medicare to maximize home health care benefits with the utmost respect for each patient's privacy and confidentiality.

Our Service

We provide patients services with  Skilled Nursing, Physical Therapy, Occupational Therapy, Speech Therapy, Home Health Aide, Medical Social Worker and Registered Dietician following a discharge from an acute care medical facility, rehabilitation hospitals well as those patients that can receive care at home instead of the hospital or another acute care provider.  See our complete list of services.
