Skilled Nursing 

 

Skilled Nursing is provided by Registered Nurses (RNs) or Licensed Vocational Nurses (LVNs)  and includes coordinating the plan of care which your physician has prescribed, monitoring vital signs, giving injections, and providing wound care and assessing, diagnosing, planning, implementing and evaluating patient care.​

Senior Therapy

Physical Therapy

​

The goal of physical therapy is to help you regain movement and strength.

Our physical therapists will visit you in your home to help you perform daily activities and progress towards your physical goals. With physical therapy, we aid with relief of muscle and joint pain, provide training for adaptive equipment such as walkers, and educate our patients and families on the patients’ condition and treatment.

Occupational Therapy

​

Occupational therapy is designed to help you regain your independence by performing daily activities by yourself. You may learn new ways to eat, put on clothes or comb your hair. Life Care Home Health's occupational therapists retrain those who have lost function due to disability, stroke, or other physical challenges. This retraining can include:

​

  • Fine Motor Coordination Training

  • Upper Extremity Strengthening & Function

  • Teaching of compensation for loss of sensation and vision​

  • Energy Conversation Techniques

Speech Therapy

​

After a stroke or accident, speech therapy can help you recover your swallowing and communication skills. We can order Speech-Language Pathologists (SLP) for the evaluation and treatment of swallowing, cognition/thinking skills and communication disorders to help your regain independence and improve your quality of life.

  • Swallow Evaluation and Treatment

  • Retraining of speech – how to produce sounds, improve quality of voice, and communicate more effectively

  • Exercises to develop and strengthen muscles used to swallow

  • Enhance Cognitive & Language Skills

Medical Social Worker​

We know physical issues aren’t your only concern when recovering from or managing an illness. Medical Social Workers can visit your home to help you with the social and emotional concerns that may accompany your health care issues.

  • Long Term Planning & Counseling

We want to help you live in a safe and clean environment. If your physical limitations or disabilities hinder you from keeping an organized space, one of our caregivers can help you out. They can manage your daily housekeeping tasks for you.
